Notice is hereby given that sealed bids will be received by The Board of Trustees, The Health and Hospital Corporation of Marion County, Indiana; dba Eskenazi Health herein after referred to as Owner, for the following:

The project consists of a phased interior renovation of a portion of the existing Burn Clinic to accommodate additional examination rooms and relocate support functions to improve the facility’s capacity.  The existing surrounding areas will remain operational throughout the duration of the construction project and maintain strict infection control. Interior selective demolition, new interior wall partitions, interior finishes, furniture, doors and hardware, and mechanical, electrical, information technology and plumbing systems assemblies are included in the scope of work.

Bids shall be based on and Work shall be performed under a stipulated sum contract for all construction and equipment.

Owner will receive bids until 3:01 p.m. local time on Thursday, August 9, 2018 at Eskenazi Health, Purchasing Department Office, 720 Eskenazi Avenue, Fifth Third Bank (FOB) Building 3rd Floor, Indianapolis, IN 46202, Attn:  Adelaide Mhlanga at which time all bids will be opened and read aloud at FOB 3rd Floor Conference Room 021A and 022B. Bids received after this time will not be accepted and will be returned unopened. Please note that the Fifth Third Bank (FOB) Building is attached to the south end of the hospital. Parking is available on site and in the parking garage.

Bids shall be delivered in a sealed envelope showing the Bidder’s name and address.

Bids shall be in full accordance with Bidding Documents, properly and completely executed, and which are on file and may be examined on or after Thursday, July 19, 2018 at 8:00 am by prospective Bidders at following locations:

Copies of the Bidding Documents are available to Bidders from reprographer (listed below).  Only complete sets of Bidding Documents will be available to bidders at cost of printing under no deposit/refund policy. Each Bidder will be required to obtain a minimum of one complete set of documents. One non-reproducible viewable set will be available electronically through the reprographer.  Use of partial sets does not waive the Prime Bidders’ responsibility to provide a complete bid.

Bid shall be executed in accordance with Bid Form, Contractor’s Bid for Public Work – Form 96 (current revision) available at http://www.in.gov/sboa/files/Form96.pdf, enclosed Supplemental Bid Form, Instructions to Bidders, and accompanied by satisfactory Bid Security of either a Bid Bond (AIA Document A310) for not less than five percent of the total base Bid amount or a certified check for not less than five percent of the total base Bid amount, payable to the Board of Trustees of the Health and Hospital Corporation of Marion County.  Deliver Bid Security in a sealed envelope showing Bidder’s name and address with the Bid.

Bidder whose principal place of business is not in the state of Indiana, shall file with its base Bid, a Bid Bond in amount described above from a company whose principal place of business is in the state of Indiana or certified check for amount described above drawn on a bank or trust company who is a member of the Federal Reserve System.

Requirements contained in Procurement Documents shall be in compliance with Bid Conditions for Marion County for equal employment opportunity.  Owner is an equal opportunity employer.

For contracts estimated to be over $300,000.00, that Bidder and expected subcontractors are required to be qualified under IC 4-13.6-4 or IC 8-23-10. If the Bidder wishes to be considered as the low bidder, it will be required to submit proof of those certifications within 24 hours of bid opening.

A pre-Bid conference will be held at the Project site on Wednesday, July 25, 2018 at 11:00 am local (Project) time.  Meet at the Eskenazi Health Hospital, Main Level Visitor Information Desk, 720 Eskenazi Avenue, Indianapolis, IN 46202. Attendance at this meeting is strongly encouraged.

The Owner has an established M/W/V/DOBE Participation Program.  

A contract will be awarded to the lowest responsible and responsive bidder, complying with the conditions of the Instructions to Bidders and Advertisement for Bids. The Owner, however, reserves the right to reject any and all Bids, and waive any informalities, discrepancies, omissions, variances, or irregularities in Bids received in its sole discretion. If a contract is awarded, it will be to the “lowest responsible and responsive bidder” in accordance with Indiana’s Public Work Projects Act, Indiana Code 36-1-12. The Bidder agrees to hold open its Bid and any and all alternate bids for a minimum of sixty (60) days from the date of the Bid opening.

Should a successful Bidder withdraw its Bid or fail to execute a satisfactory contract within ten days after notice of acceptance of Bid, Owner may declare Bidder’s Bid Security forfeited as liquidated damages, and not a penalty.

Successful Bidder shall furnish a Performance Bond (AIA Document A312-2010) and Payment Bond (AIA Document A312-2010) each in amount equal to one hundred percent of Contract Sum with an approved surety company and said bonds shall remain in full force and effect for a period of one year after the Owner’s final settlement with the successful Bidder.
